Can turmeric really erase years of knee pain?

An Instagram post cites clinical trials to back its nightly turmeric-paste remedy. But what did those studies actually test?

CLAIM: An Instagram post by longevity physician Dr Darshan Shah claims warm turmeric paste on knees nightly can break down years of joint inflammation, enabling pain-free movement. It cites “15 randomized trials, 1,670 patients” showing curcuminoids significantly reduce knee pain versus placebo, rivalling anti-inflammatory drugs, calling this “not folklore.”

FACT: Turmeric supplements, taken orally, may modestly help knee osteoarthritis pain when tested in clinical trials, sometimes matching common painkillers. But a warm paste applied to skin is a different matter entirely. Poor skin absorption means it likely cannot reach the joint in meaningful amounts, and no topical remedy reverses years of cartilage damage overnight. 

 

An Instagram post claims that a warm turmeric paste applied to the knees at night can undo years of accumulated joint inflammation, promising relief so complete that people can walk downstairs without the familiar wince of pain.

The account behind the post, drshahlongevity, is run by Dr Darshan Shah, a longevity physician and founder of the health clinic NextHealth. The post frames the remedy not as a folk tip but as something backed by clinical research. 

“Turmeric contains curcuminoids that have been properly tested for knee osteoarthritis across 15 randomized trials covering 1,670 patients,” the caption read, lending the claim an air of scientific authority.

The account has amassed over a million followers, and the post itself has drawn more than 2,500 likes and over 170 comments, many from users asking how to prepare the paste or sharing their own knee pain struggles.

The caption goes further, stating that curcuminoids “significantly reduced knee pain compared to placebo” in these trials, and that in some studies, the compound performed comparably to anti-inflammatory drugs. To pre-empt scepticism, the post explicitly says, “This is not folklore,” positioning the paste as an evidence-based fix rather than a grandmother’s remedy repackaged for Instagram.

The framing is notable as it states that real trial numbers are cited upfront, which makes the claim feel credible before it ever specifies whether those trials tested a paste applied to skin, or something else entirely.

What are studies saying?

A systematic review published in BMJ Open Sport and Exercise Medicine in 2021 looked closely at whether turmeric or curcumin supplements actually help people with knee osteoarthritis. Researchers examined ten randomised controlled trials, screening over five thousand studies before settling on ones with strong methodology.

All ten trials found that turmeric therapy improved pain and movement compared to when participants started. Three of these trials went a step further and compared turmeric directly with common anti-inflammatory painkillers, finding no meaningful difference in results between the two.

The review noted, “There appears to be a benefit of turmeric on knee OA pain and function.” However, the authors were careful to add that questions around the right dose, frequency and form of turmeric remain unresolved.

Importantly, none of the ten studies reviewed tested a paste applied to the skin. Every trial involved turmeric or curcumin taken as a supplement, swallowed rather than rubbed on.

A 2025 systematic review and network meta-analysis published in BMC Complementary Medicine and Therapies examined seventeen randomised controlled trials to compare different turmeric preparations for knee osteoarthritis pain. 

Researchers searched major databases up to August 2024, looking specifically at how various forms of turmeric measured up against placebo and standard painkillers.

The findings showed that all turmeric preparations significantly reduced pain, measured using a standard osteoarthritis pain index. Preparations enhanced for better absorption showed a 30% reduction in pain compared to placebo, while combining these enhanced preparations with conventional drugs led to a 70% reduction in pain compared to drugs alone.

“All turmeric preparations appear to be effective in reducing knee OA pain when used as monotherapy compared to placebo,” the study noted.

However, the researchers flagged an important caveat. The certainty of evidence was rated low, and they called for better-designed, longer-term studies to confirm these results. As with earlier reviews, none of the seventeen trials tested turmeric as a paste applied to the skin.

What are experts suggesting?

Dr Chethana D, senior consultant of rheumatology at Aster CMI Hospital, Bangalore, said the claim should not be taken at face value. She explained that osteoarthritis is a long-term condition involving cartilage wear, bone changes and joint inflammation, and no paste applied to the skin can reverse that structural damage. 

“Turmeric paste should not be described as something that can break down or remove years of inflammation in an arthritic knee,” she said, adding that the warmth from the application may temporarily ease stiffness, but that this is different from treating the disease itself.

Dr Chethana pointed out that skin acts as a barrier, and curcumin’s poor absorption means little of it likely reaches the joint in meaningful amounts when applied as a homemade paste. She noted that “specially formulated topical products designed for penetration cannot be compared to turmeric stirred at home.” 

She also flagged the trial citation itself. Clinical research on curcumin, she said, generally involves oral supplements, not skin application, and the formulation, dose and method of use all shape how findings apply. “People should check exactly what treatment those trials actually evaluated before accepting the claim,” she said.

Dr Chethana added that no topical remedy can reverse cartilage damage overnight, and that meaningful relief instead comes from consistent measures: low-impact exercise, muscle strengthening around the knee, weight management, physiotherapy, and medically approved treatments where needed.
